We understand that how we take care of patients is directly related to how we care for our employees! Reviews the spiritual needs assessment and provides spiritual support to the patient and family when referred by the hospice social worker, RN, hospice team coordinator or other member of the interdisciplinary team. Services will be provided in SNFs, IPUs and homes in Brooklyn, NY. Qualifications: A Baccalaureate Degree in ministry, psychology or counseling from an accredited university or divinity institution. A master’s degree is preferred, 1+ years of supervised experience in counseling in a medical, educational or religious institution, Exceptional oral and written communication skills, Ability to work with limited supervision, Strong interpersonal skills, Collaborative and team-oriented personality, Ability to work with a diverse patient population, Reliable motor vehicle and valid NYS license required & Computer proficiency and working knowledge of EMR a plus. Compensation: $50 pay per visit, $25 an hour for orientation/training (Mandatory 2 weeks of orientation), 401k participation & Mileage and parking reimbursement during patient visits.
