The Jewish Care and Support Program at Gilchrist provides culturally sensitive and competent care to Jewish patients and/or their families within the continuum of Gilchrist Services.  This program recognizes our patient’s and family’s religious and cultural beliefs. The rabbi/cantor support consultant will be tasked with providing culturally sensitive care to Jewish patients from a wide variety of religious/traditional backgrounds, including – when needed – after hours, on weekends, and on national holidays. The rabbi/cantor will be a part of an on-call rotation and will help Jewish individuals and their families attain the highest quality of life when experiencing a serious illness, while observing Jewish laws, tradition, and values. Because of the on call rabbi/cantor program,  patients and their families will take comfort in the knowledge that they will have the opportunity to have a rabbi or cantor available during their time of greatest need. This position will work collaboratively with a team of rabbis and cantors who will be called upon through a weekly shift schedule. The candidate must be ordained as a rabbi or cantor from a recognized institution and have familiarity with end-of-life care. The candidate must have 3-5 years’ experience working with and guiding families of diverse Jewish religious observances including the Jewish secular community. Experience: Experience in caring for a diverse Jewish community, Experience in providing culturally sensitive spiritual care to patients and their families with serious illness and end of life care, Experience in facilitating end-of-life conversations with families, Experience in documenting in Electronic Medical Records & Experience in collaborating with an interdisciplinary team. Knowledge, Skills, Abilities: Knowledgeable about Jewish laws, traditions, and values, Understands the importance of timely outreach calls to every Jewish family within 24 hours of admissions, Serves as the connection between the patient/family and the care team to ensure that their specific values and cultural sensitivities are being met, Is available to family when questions or concerns arise and assure that they are not alone, Provides education and guidance to Gilchrist Care Team about the differences between clinically indicated and religious /cultural indications, Communicates a sense of comfort through Jewish liturgy, traditions, common language, traditional prayers, and traditional songs, Serves as the liaison with the patient/family and their synagogue clergy, Maintains relationships with families after their care at Gilchrist to provide resources and assistance with grief and bereavement, Provides sensitive Holocaust Survivor Care, Communicates with team manager and team chaplain regarding patient care planning in a timely manner, Responds to all inquiries for spiritual support within 30 minutes, Provides timely documentation; within the same day of contact/visit, Ability to work with the program manager to develop community partnerships with educational, religious and community organizations, Time management skills and ability to manage multiple priorities, Ability to function with high level of autonomy & Provides spiritual care and support after hours including weekends and holidays. Rabbinic or cantorial ordination from an accredited institution. Physical and medical ability to participate in visitations in multiple care settings; home, residential care community, inpatient centers, etc. Work is performed in a variety of care settings and wherever patients are being cared for in the community. Reliable transportation, valid driver’s license, and automobile insurance. Ability to work evening or weekend hours as needed to provide spiritual care and family support. Paperwork Required. Standard precaution policy and procedures are applicable to this job as relevant to current CDC measures. Employee has knowledge and understanding of patient safety as it relates to the job duties.
